1. Duly accomplished PDAO Application form

4. Old/ Expired Disability ID
REPLACEMENT
1. Duly accomplished PDAO Replacement form
2. Latest 2x2 ID picture
3. Affidavit of Loss
CHANGE OF STATUS
1. Duly accomplished PDAO Replacement form
2. If newly married - Photocopy of Marriage Certificate
3. If annulled - Annotated Marriage Certificate issued by PSA
4. If widowed - Death Certificate of Spouse
CHANGE OF ADDRESS
1. Duly accomplished PDAO Replacement form
2. Original copy of Barangay Clearance
3. Old Disability ID
