1. Duly accomplished PDAO Application form
2. Duly accomplished DOH - PRPWD form 
3. Latest 2x2 ID picture
4. Old/ Expired Disability ID


REPLACEMENT

1. Duly accomplished PDAO Replacement form
2. Latest 2x2 ID picture
3. Affidavit of Loss

1. Duly accomplished PDAO Replacement form
2. If newly married - Photocopy of Marriage Certificate
3. If annulled - Annotated Marriage Certificate issued by PSA

1. Duly accomplished PDAO Replacement form
2. Original copy of Barangay Clearance
3. Old Disability ID

Barangay Association of Persons with Disabilities in Taguig City (BAPDTC)
  • Available in each barangay.

Barangay Affairs Office (BAO) Satellite Office
  • Available in each barangay.
