- Photocopy of Barangay Clearance or NBI Clearance
- Photocopy of a valid Government-Issued ID with three specimen signatures
- Photocopy of Medical Certificate or Barangay Health Certificate (stating "Physically Fit for TESDA Training")
- Two (2) pieces of 1x1 ID pictures (white background, formal attire)
Additionally, applicants must meet the following conditional requirements:
- Ability to communicate both orally and in writing.
- Ability to perform basic mathematical computation.
- At least a holder of a Student Permit or LTO license (Restriction Code 1).
