  • Barangay Certificate of Indigency: Secure this from your Barangay Hall’s main desk before sitting down with the CSWDO social worker. It must explicitly state that the patient is an indigent resident needing medical financial assistance.

  • Valid Government-issued IDs: Bring the original and photocopies of the patient's valid ID. If you are processing the request on behalf of the patient, bring your own valid ID and a signed Authorization Letter.
