A Social Case Study Report (SCSR) is a formal evaluation compiled by a licensed social worker. It acts as official evidence that an individual or family is facing financial distress and lacks the economic capability to pay for critical healthcare services. If you are applying for healthcare assistance from the Taguig Medical Assistance Office (MAO), public hospitals, or charities, this document is strictly required.

The City Social Welfare and Development Office (CSWDO), located at the Taguig City Hall, has implemented updated structural requirements. Here is a comprehensive guide to completing your application smoothly.


Required Documents

The CSWDO divides its required checklist into primary medical/billing documentation and structural local government certifications. Prepare both the original documents and two (2) clear photocopies of each item before visiting the window:

Clinical Abstract or Medical Certificate: Must be officially signed by the attending medical professional, carrying their explicit PRC license number, and dated within the past three (3) months.

Protocol / Price Quotation: An itemized billing estimate or diagnostic fee slip from the hospital or laboratory detailing exactly how much the procedure or test costs.

Proof of Billing: Your latest residential electric bill (MERALCO) or water bill. The address listed on the statement must explicitly align with your stated place of residence in Taguig.

Photocopy of Valid Government-Issued ID: A clear copy of an unexpired photo ID belonging to the Patient or the designated Claimant (the immediate family representative processing the request).


Conditional Institutional Requirements


Certification from the Assessor’s Office: A certificate verifying the patient or claimant’s property holdings (or lack thereof). This can be obtained at the 1st Floor of the Main Taguig City Hall building.

Certification from the Business Permit and Licensing Office (BPLO): Documentation confirming that the patient/claimant does not operate a registered commercial business in the city. This is obtained on the 4th Floor of the New Taguig City Hall building.

PSA Birth Certificate: Required only if the patient is a minor, to legally establish kinship between the child and the claimant parent/guardian.

Referral Letter: An institutional referral document from a external partner agency, foundation, or welfare group, if applicable.



Step-by-Step Filing Procedure

Because the process requires cross-department clearances inside the Taguig City Hall compound, follow these sequential steps to save time:


Step 1: Consolidate Your Medical and Billing Paperwork

Secure your signed Medical Certificate, itemized Price Quotation from the lab or medical center, a copy of your utility bill, and the valid IDs of the parties involved.


Step 2: Obtain Your Assessor and BPLO Clearances

Before heading to the CSWDO department counter, clear your institutional certifications within the main City Hall building:

  • Proceed to the 1st Floor Assessor's Office to pick up your property certification.

  • Go to the new Taguig City Hall along Cayetano Blvd. at the 4th Floor Business Permit and Licensing Office (BPLO) to pick up your business registry clearance.


Step 3: Submit Your Application Packet to the CSWDO

Head to the City Social Welfare and Development Office (CSWDO) desk at the Old City Hall compound. Hand over your complete document packet to the intake officer. The staff will log your application, assign your file to an available social worker, and record the name of the patient and claimant on your tracking slip.


Step 4: Undergo the Mandatory Community Interview

The CSWDO officially mandates that every application for a Social Case Study Report is subject to a Community Interview / Field Assessment. Step 5: Document Issuance (1–3 Days Processing Window)

Due to the mandatory community interview and verification steps, the Social Case Study Report is not issued instantly. The official processing timeline for releasing the finished document spans 1 to 3 working days from your submission date. Once ready, pick up your signed report and attach it directly to your MAO document packet.
