Requirements
- ✓ Photocopy of Birth Certificate
- ✓ Photocopy of the Marriage Certificate (if applicable)
- ✓ Photocopy of Previous School Record (HS or College) – Diploma or TOR
- ✓ Photocopy of Brgy. Clearance or NBI Clearance
- ✓ Latest ID Pictures
- 4 pcs 1×1 picture (White background, formal attire)
- ✓ Photocopy of valid Gov’t Issued ID with 3 specimen signatures
- ✓ Photocopy of Medical Certificate or Brgy. Health Certificate
- ✓ Certificate of Indigency
Conditional Requirements
- ✓Must be High School graduate
- ✓18 years old and above
- ✓Must have a good moral character
- ✓Must be physically and mentally fit as per MLC 2006 Regulation 1.2 – Medical Certificate
- ✓Can communicate in English both oral and written
